An eligibility screening must be completed before scheduling an appointment.
SCREENING HOURS:
EVERY TUESDAY
9 am to 12 pm
You may walk-in during these hours to complete an eligibility screening. Once you complete this screening and qualify, you will be given an appointment.
You MUST bring a picture i.d. and any of the following proofs of income that you have:
- Income Tax Return (Form 1040) or (all) W2 forms from last year.
